Care 360°: Full-Circle Health Care

We’re committed to making your health care journey as easy to navigate as possible, from the time you enter our doors through being discharged to your home. Our Care 360° Program eases the transition for patients going from the hospital, or emergency room, to home or to another care facility.

Our Transition Specialists will connect you with resources needed for a successful recovery, even before you’re discharged from the hospital. This program ensures a seamless experience of continuity of care for you, and your family, at no additional cost for the program — just your normal co-payments and/or deductibles. We’ll take care of all the details of your transition care plan so that you can focus on what matters most: healing.

Transition Specialists: Your Personal Guides to Next Steps

It can feel overwhelming to be in the hospital, or emergency room, and we want you to focus on getting better rather than worrying about your next steps. Rest assured that before you even leave, an AdventHealth Care 360° Transition Specialist will be there to coordinate those important next steps with you.

Whether you’re transitioning back to your home, a rehabilitation facility or a skilled nursing facility, our team can help. Your Transition Specialist can make transportation arrangements for you, help you get your medications, schedule follow-up medical appointments, communicate with your care team and more. We want to help you regain your sense of wellness after your hospital stay.

Your Transition Specialist can also find programs that may assist with paying for prescriptions. In some locations, we can arrange for delivery of your medications before you leave the hospital or have them delivered to your house. Transition Specialists are here to partner with you and take the pressure off with every detail in your recovery, providing concierge-level care personalized to you and your needs.

Care Advocacy Through the AdventHealth App 

Your health care and medical records are now at your fingertips, literally, along with a dedicated Care Advocate, also known as your ‘friend in health care.’ The AdventHealth app is designed to help manage important aspects of your family’s health, including:

Find a Physician

Searching for a primary care physician, or medical specialist, is easy with the advanced physician-finder tool. Search by zip code, physician’s name, specialty or location. You’ll find doctor biographies, areas of specialty and educational videos that can help you choose the physician who’s right for you.

Schedule an Appointment
Now you can schedule, or request, an appointment with your primary care provider, a specialist, a medical imaging facility or a laboratory, right from your computer or smartphone. We offer weekend and evening hours in some locations.

Access Medical Records
In the app, you can create personalized profiles for your family health history that can be viewed throughout our network. You’ll have direct access to your medical records, including lab results, medical images, surgeries, physician notes and more.

Virtual Urgent Care
By downloading the AdventHealth app, you can conveniently get a medical video visit with an urgent care doctor from the comfort of your own home or on the go. Once a physician diagnoses your condition, he, or she, can submit a prescription to the pharmacy of your choice. Common conditions treated through virtual visits include:

  • Conjunctivitis or pink eye
  • Coughs, colds, bronchitis and flu
  • Seasonal allergies
  • Sinus and upper respiratory infections

Communicate with Your Care Advocate
Have a follow-up question for your doctor or want to send the office staff a message without having to make a phone call? Connecting with your personal Care Advocate through the app is easy with convenient in-app messaging.

Prescription Refill Requests
Are your medications running low? No problem. Just use the app to make a quick, and easy, refill request and be on your way.

Online Bill Pay
View paper bills, and make payments, all in one central location. You’ll never have to worry about losing track of a paper bill again.
